New Patients

We can’t wait to meet and welcome you to our office! Our team is committed to making your visit as pleasant and informative as possible.

Prior to your first visit, please print and fill out all of the documents in the New Patient Packet Forms. Bring the completed forms with you to speed up your check-in process. Copays are also due prior to being seen by one of our providers.

During your first visit, a medical assistant will take some information and a digital x-ray if necessary. You will then be seen by one of our providers, who will review your medical history, primary issue and present their diagnosis and treatment plan. Many procedures can be performed in the office that day.

Be sure to arrive early so that we can complete your initial paperwork. Also, please bring the following:

  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records

To have the office send your medical record to another physician’s office, please fill out this Authorization form and send it back to us.

Agreement to Pay - To set up automatic recurring payments, click on the blue “Pay Now” button above and choose the “Recurring Payment” option. You can also sign and return this Agreement to Pay form and the office will set it up for you.

Insurance and Payment Information

You are responsible for co-payments at the time of service. You are also responsible for charges that are not covered by your insurance plan. If you have questions regarding your bill or which insurance plans we accept, please call us at (618) 277-0001. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
